Centers for Disease Control (CDC): Healthy Communities Program

The Community Health Resources Database plans, implements, and evaluates community health interventions and programs that address chronic diseases. Developed by CDC’s Community Health and Program Services Branch, the database includes links to hundreds of useful planning guides, evaluation frameworks, communications materials, health risk factors data and statistics, fact sheets, scientific articles, key reports, and state and local program contact information.

The mission of the Data Resource Center (DRC) is to take the voices of parents, gathered through the National Survey of Children’s Health (NSCH) and the National Survey of Children with Special Health Care Needs (NS-CSHCN), and share the results through this free online resource. Easy access to children's health data allows researchers, policymakers, family advocates and consumers to work together to promote a higher quality health care system for children, youth and families.

The Nassau County Department of Health supports the collection and dissemination of health data for public health research and education. The department regularly publishes health information in the Community Health Assessment, the Annual Report, and in the Municipal Public Health Services Plan and Performance Report. The principal agency that produces general-purpose health data. NCHS is responsible for the collection, maintenance, analysis, and dissemination of statistics on the nature and extent of the health, illness, and disability of the U.S. population; the impact of illness and disability on the economy; the effects of environmental, social, and other health hazards and health care costs and financing.

The National Vital Statistics System is the oldest and most successful example of inter-governmental data sharing in Public Health and the shared relationships, standards, and procedures form the mechanism by which NCHS collects and disseminates the Nation's official vital statistics. These data are provided through contracts between NCHS and vital registration systems operated in the various jurisdictions legally responsible for the registration of vital events – births, deaths, marriages, divorces, and fetal deaths.

The Health Department gathers data and statistics from New Yorkers and uses them to inform the public and health professionals about the health of the City over time, to establish department priorities and to shape health policy at all levels of government.

The Suffolk County Department of Health Services offers a wide range of programs and services to the residents of Suffolk County. Staff in the department’s nine divisions includes health-care professionals, engineers, educators, entomologists, forensic examiners, sanitarians and more.

The Department of Health and Human Services (HHS) is the United States government’s principal agency for protecting the health of all Americans and providing essential human services, especially for those who are least able to help themselves.
